A trim size is not a page size. The page is the trim plus the bleed on every side, so an A4 job with 3 mm of bleed is a 216 x 303 mm PDF - 612.283 x 858.898 pt, because a point is exactly 1/72 inch and nothing metric is a round number of them. Anything that offers to fit that to A4 scales it to 97.22%: the artwork shrinks, every element moves inward, and the crop marks land 2.92 mm inside the trim rather than on it. Creep eats the bleed too - 64 pages stitched on 0.1 mm stock creeps 1.5 mm, half a 3 mm bleed. And how you butt the pages decides the N-up rather than how big they are: on SRA3 an A5 goes 4-up with shared bleeds and 2-up with separate ones. Five lanes over one sheet, plus a free browser-side engine that computes every box in millimetres and points and imposes the job both ways round.
